Dinner Meeting Reservation Form
This form is to be used only during the week preceding the monthly meetings which occur on the third Tuesday of each month from September to May. Reservations should be made by noon on the Friday preceding the meeting. Cancellations must be submitted by 10 a.m. on the Monday before the meeting.

Please include me for the dinner count for Tuesday’s meeting. If I do not notify Opal Brown by Monday of a cancellation, I will be responsible for the $16 cost of the meal whether or not I attend.
